In Case of a Personal / Travel Accident

Report the incident as soon as possible to Reliance Surety & Insurance Co., Inc. c/o Claims Department and submit the following documents:

  1. For Medical Reimbursement Claims:
    • Police report
    • Original bills and official receipts
    • Result of medical and laboratory examinations
    • Doctor’s prescriptions (to support medicines bought)
    • Claimant’s and physician’s statements
    • Certified copy of operating room record (for dismemberment / disability)
  2. For Death Claims:
    • Police report
    • Original bills and official receipts
    • Certified copy of operating room record
    • Results of medical and laboratory examinations
    • Death certificate (authenticated by the Civil Registrar)
    • Birth certificate
    • Proof of relationship to beneficiary (i.e. marriage / birth certificates, etc.)
    • Claimant’s and physician’s statements
